PART A (General Information)

1. Is this the first time you are attending the Asia Microfinance Forum?

2. The Asia Microfinance Forum will be conducted in English, however it is intended that translation services in both Sinhala and Tamil will be provided for certain sessions. Please indicate if you would like translation.

3. Do you have any dietary requirements?

4. The general session of the conference will begin on 13 October, however an opening dinner will be held the prior evening on the 12th. Will you be attending the opening dinner to be held on 12 October at the conference venue?

5. Will there be anyone accompanying you who will not be attending the conference, but who may require transportation and accommodation services? (ie. spouse)

Please note that only registered delegates will be permitted to join conference meals (ie opening dinner and daily lunches).

Refund Policy: Participants who cancel their registration on or before September 15, 2010 will be refunded 100% of PAID registration fees less a USD 50 administrative/processing fee. Participants who cancel their registration after September 15, 2010 are not entitled to a refund.
